HomeMed’s planned service is intended for eligible community-based patients in Broadmeadows, Dallas and Coolaroo. An enquiry is only a starting point: an eligible medical practitioner must determine whether an HMR is appropriate, and appointment availability is confirmed individually.

With the patient’s consent, a family member may join the interview. Planned interviews are available in English, Urdu or Hindi; other language-support needs can be discussed before an appointment.

How it works, from your side

2. Eligible referral. Most referrals come from the person’s GP; specified specialists can also refer.
3. Home interview and report. Once the service opens, the interview is planned for the patient’s home, with a relative present when the patient wants them there. The report goes to the referrer and, when different, the regular GP.
