The pharmacist who comes to the kitchen table
I’m Usama Yousuf, an AHPRA-registered pharmacist based in Craigieburn. I’ve worked across Melbourne’s community pharmacies, and HomeMed Reviews is the service I built around one observation: the most important medication problems are invisible in the pharmacy and the consulting room — they live at home, in the kitchen drawer, the bedside table, and the Webster pack nobody opens.
A Home Medicines Review is my answer to that. It’s a government-funded program that’s been part of Australian primary care for over two decades: your GP refers, I visit, and your doctor gets a clear picture of what’s actually happening with the medicines — not just what’s on the list.
How I work: every interview is conducted by me, and every report is written by me — personally, within three business days of the visit. Reviews are unhurried; most run 45 to 60 minutes, and family are welcome to sit in when the person being reviewed wants them there.
Languages: I consult in English, Urdu and Hindi — whichever language the person thinks in. For many families in Melbourne’s north, that’s the difference between a polite conversation and a real one.
