This page explains how personal information is handled when you get in touch about a Home Medicines Review, whether through the website form, by phone, or by email.

What I collect

When you contact me — through the request form, a phone call, or an email — I collect the details you provide: your name, contact number, email if given, your family member’s suburb, and any brief notes about their situation. If a review goes ahead, I also collect and record the clinical information needed to complete it, such as their current medicines.

Why I collect it

This information is used for one purpose only: to arrange and carry out the Home Medicines Review you’ve asked about. It is never used for marketing, and I don’t build mailing lists or profiles from it.

Who sees it

The only external party your information is shared with is the GP practice directly involved in that specific review — because the review is a GP referral, and the report has to go back to your family member’s own doctor. Your details are never shared, disclosed or sold to anyone else, for any reason.

How long I keep it

Clinical records from a completed review are kept in line with the record-keeping requirements of the Home Medicines Review program and my professional obligations as a registered pharmacist. Enquiry details from people who don’t go on to have a review are kept only as long as needed to respond to that enquiry.

Access and correction

You’re welcome to ask what information I hold about you or your family member, or to have anything corrected, at any time — email usama@homemedreviews.com.au and I’ll respond personally.

Complaints

If you have a concern about how your information has been handled, please contact me directly first so I can address it. You can also raise a privacy complaint with the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au.

This policy reflects how HomeMed Reviews actually operates, as a sole-trader pharmacy practice, and is handled in line with the Privacy Act 1988 (Cth).
