Prompts, not criteria — the clinical judgment is yours. The patients below are where an HMR most often surfaces problems that never show up in the consulting room.
Generate the HMR referral through your practice software as usual — you claim MBS item 900 when you complete the medication management plan after receiving the report. A template setup guide for Bp Premier and MedicalDirector is available below.
How it works
From referral to report
A structured clinical service with a program timeframe of 90 days — my working aim is two weeks.
Through your practice software under MBS item 900, or using the referral form below.
I contact your patient, explain the service, and book a time that suits them. You’re advised of the details.
45–60 minutes at the kitchen table, with signed consent — adherence, interactions, duplications, device technique, and the practical problems that only show up at home.
A concise, prioritised report within three business days of the interview — by secure messaging or encrypted email, however your practice prefers — for the medication management plan discussion.

Three ways to refer
Set up the template and download the form now — referrals are accepted from the day the credential is issued, and I’ll confirm that date with your practice by email.Whichever is easiest for your practice. Every referral is acknowledged by email the same day it’s received.

Refer under MBS item 900 as usual. Set up the HomeMed template once — MedicalDirector practices use the built-in DMMR template (~2 minutes); Bp Premier takes ~10 with the guide.

Practicalities
What does it cost the patient or the practice?
Nothing. Home Medicines Reviews are funded under the Australian Government’s Community Pharmacy Programs. There is no charge to the patient or the practice.
How quickly will my patient be seen?
The program allows interviews up to 90 days from referral; my working aim is within two weeks. If I ever can’t service a referral promptly, I’ll contact your practice within two weeks of receiving it — you’re never left wondering.
How is patient information handled?
Health information is handled under the Privacy Act. Interviews take place with the patient’s signed consent — which also covers any recording — records are retained as the program requires, and nothing is ever used to train AI systems. Reports are written by me personally.
How do reports come back to us?
Within three business days of the interview, by your practice’s preferred channel — secure messaging or encrypted email — so the report files straight into the patient record for your item 900 medication management plan consult.
What if my patient declines or can’t be reached?
If the patient declines, or I can’t reach them after three attempts over two weeks, I let your practice know in writing — you always find out what happened to a referral, and it never silently dies.
Do you service residential aged care?
HMRs are for people living in the community — their own home, or a family member’s. Residents of aged-care facilities are covered by a separate program (RMMR); if that’s your patient, get in touch and I’ll point you the right way.
